I am writing to inform you of an upcoming change in our practice. Our nurse practitioner, Keegan Scofield, will be leaving our practice to join the Guthrie Clinic Department of Family Medicine. His departure is entirely amicable, and this transitionwill allow him to pursue his career goals further.

Keegan has been an integral part of our team, providing exceptional care to our patients. If he has been your primary provider, you are welcome to continue your care with him at Guthrie Clinic Centerway in Corning, where he will begin seeing patients in April. If you choose to follow Keegan, we will gladly assist with transferring your medical records upon request.

For those wishing to remain with our practice, please rest assured that we remain fully committed to providing the same high-quality, personalized care you trust. We are here to answer any questions and support you throughout this transition.
